Solutions to frequently encountered problems with physical securities and folio management
Folios are frozen if PAN, KYC, nomination, contact details, bank details, or specimen signature are missing as of October 1, 2023. Submit Form ISR-1 with all required documents to unfreeze.
Download ISR-1 βUpdate your bank details with Capifide using Form ISR-1. Ensure ECS / NEFT mandate is active. Unpaid dividends may be in Unclaimed Dividend Account or transferred to IEPF after 7 years.
Contact Us βSubmit Form ISR-4 with a notarized Indemnity Bond and Affidavit. For securities above βΉ5 Lakhs, FIR / newspaper advertisement is also required. Duplicate will be issued as a Letter of Confirmation for demat.
Download ISR-4 βFor minor mismatch, Capifide sends a notice giving 15 days to object. For major mismatch, submit cancelled cheque and banker's attestation via Form ISR-2, or visit our office for in-person verification.
Download ISR-2 βFor minor name mismatch, submit any two identity documents explaining the difference (Aadhaar, Passport, PAN etc.). For major mismatch or name change (marriage/divorce), provide Marriage Certificate or Gazette publication.
Upon death of a holder, submit Form ISR-5 with Death Certificate, KYC of surviving holder / nominee / legal heir. Transmission will be processed in demat mode only (no physical certificates).
Download ISR-5 βPhysical transfer of listed company securities has been discontinued from April 1, 2019. Investors must open a demat account, convert physical securities to demat, and then transfer through the depository system.
If securities or dividends have been transferred to the Investor Education and Protection Fund (IEPF), approach the IEPF Authority at iepf.gov.in for claims. These matters fall outside SEBI's regulatory purview.
