Documents for Proof of Identity

Any one of the following documents is acceptable as proof of identity for shareholder service requests submitted to Capifide:

  • 1Unique Identification Number (UID) — Aadhaar
  • 2Valid Passport
  • 3Driving License in Smart Card form, Book form or copy of digital form
  • 4Identity card / document with applicant's Photo, issued by Central / State Government, Statutory Authorities, PSUs, Scheduled Commercial Banks, or Public Financial Institutions (duly attested by employer)
  • 5PAN Card with photograph
  • 6Marriage Certificate
  • 7Divorce Decree
Residence Proof

Documents for Proof of Address

Any one of the following documents is acceptable as proof of address / residence:

  • 1Unique Identification Number (UID) — Aadhaar
  • 2Valid Passport / Registered Lease or Sale Agreement of Residence / Driving License
  • 3Flat Maintenance Bill accompanied with additional self-attested copy of Identity Proof
  • 4Utility Bills (Telephone / Electricity / Gas) — not more than 3 months old
  • 5Identity card / document with address issued by Government / Statutory / PSU / Bank / Financial Institution (duly attested)
  • 6Proof of address in the name of the spouse, with self-attested Identity Proof of the spouse
  • 7Client Master List (CML) of the Demat Account of the holder / claimant, provided by the Depository Participant
  • 8Only for FII / sub-account: Power of Attorney given by FII / sub-account to Custodians (duly notarized / apostilled or consularised)

All documents must be self-attested by the holder with date. Capifide does not require notarization or affidavit for standard service requests. Original documents may be required for In-Person Verification (IPV).
